Make sure you are in the right month, then click the  REQUEST AN APPOINTMENT button and complete the pop up form.  Hover your mouse over the ?, to see more details.

Keep in mind, the appointments you see, don't include travel time.  To allow for that, google the distance  between the zip code of the appt before yours and your appt zip code.  Leave that amount of time between the appointments. If there is no appointment before yours, use zip code 63376.

Lastly, after I accept the request, it will appear on the calendar and you will receive a text and email to confirm.
